动态三维空间、时间相关图像技术与超声心动图诊断胎儿室间隔缺损的对比研究

摘    要:目的探讨动态三维超声心动图空间、时间相关图像(3D—STIC)技术评价胎儿室间隔缺损(VSD)的临床价值。方法超声心动图检查2082例胎儿,疑诊为VSD的17例胎儿行STIC检查,并测量缺损大小;比较两种方法诊断VSD及测量缺损大小的差异。结果产后证实18例VSD,超声心动图诊断17例,3D—STIC诊断13例。超声心动图对胎儿VSD诊断敏感性(94.44%)高于3D—STIC(72.22%),差异有统计学意义(P〈0.05),而两种方法诊断的准确性、特异性无统计学差异(P〉0.05);超声心动图判断VSD最大内径小于3D—STIC(P〈0.01)。结论3D—STIC可直观地显示胎儿VSD的大小及形状,但敏感性不及超声心动图。

Abstract:Objective To explore the clinical value of dynamic three - dimensional echocardiography, spatiotemporal image correlation (STIC) in evaluating fetal ventricular septal defect (VSD). Methods Two thousand and eighty - two fetus were examined by echocardiography ( ECG ) , 17 cases of suspected VSD were examined by STIC, and the defect size was measured. While the difference of VSD diagnosis and defect size measurement in two methods were compared. Results Eighteen cases of VSD were confirmed after parturition, 17 cases were diagnosed by ECG, 13 cases were diagnosed by 3D - STIC. The sensitivity of fetal VSD in ECG (94.44%) was higher than that in 3D - STIC (72.22%), there was significant difference between the two methods (P 〈 0.05 ) , while there was no significant difference of accuracy and specificity in the two methods (P 〉 0.05 ). The maximum diameter of VSD by ECG was lower than that by 3D - STIC (P 〈 0.01 ). Conclusion 3D - STIC can display the size and shape of VSD in fetus directly, while the sensitivity is not higher than ECG.
