改良超滤法在婴幼儿体外循环中的作用

摘    要:目的研究改良超滤法浓缩血液的效果及其对体内部分细胞因子(TNF、IL-8、ET)的影响。方法40例小儿先心病患者,随机分为两组(实验组和对照组),每组20例。体外循环中维持血球压积20%,胶体渗透压1.33kPa左右。实验组病人在停机后,拔管前行改良法超滤。结果超滤后,病人的血细胞及蛋白等大分子物质的浓度明显提高,而IL-8、ET和尿素氮、肌酐这些中小分子物质的浓度经白蛋白校正后明显降低,TNF浓度无明显变化。结论应用改良超滤法可以迅速浓缩血液中的细胞和蛋白,在滤出水份同时,还能有效的去除多种有害中分子物质。

Abstract:Objective To evaluate the efficacy of modified ultrafiltration on concentrating diluted blood and studied the change of concentrations of some cytokins.Methods Forty children affected with congenital heart disease was divided into modified ultrafiltration group (MUF group, n=20) and control group (C group, n=20) randomly. Hematocrit at about 20% and colloid pressure at about 1.33kPa were maintained during the cardiopulmonary bypass. In MUF group, modified ultrafiltration was used after cardiopulmonary bypass. Results After ultrafiltration, hematocrit and the concentrations of macromolecular substances such as the concentration of IL - 8. ET. BUN and Cr were decreased significantly, but the concentration of TNF did not change obviously. Conclusion Using modified ultrafiltration after cardiopulmonary bypass can concentrate blood cell and protein swiftly, and harmful medium molecular substances were filtered out at the same time.
