摘    要:脑干是生命中枢所在部位,也是神经中枢的“中枢””’,该部位肿瘤手术风险大,治疗效果差。随着显微外科和影像学等技术的发展,现在脑干肿瘤手术取得很大的进展。如何在尽可能切除脑干肿瘤的同时,及时察觉和避免损伤正常脑干组织,使手术更加安全,除了应用显微外科等...

关 键 词:目的探索脑干肿瘤手术中脑干听诱发电位(BAEP)变化规律及监护作用,从而降低残死率,提高治愈率。方法脑干及其周围肿瘤患者5例,从术前至术终的每个手术阶段进行监护,并对2000多个波形数据作统计分析。结果术中牵拉、分离、挤压均可引起BAEP的Ⅰ、Ⅲ、Ⅴ波潜伏期和波间期延长,振幅降低,波形分化差等变化。数据显示:手术操作对BAEP的变化有显著性影响(P<0.01),术侧与非术侧的BAEP差别存在着非常显著的差异(P<0.01)。结论在脑干部位手术中,BAEP能敏感地向术者反映脑干功能受损的状况,用单耳刺激、单耳或双耳记录的BAEP能可靠地进行监护。

Abstract:Objective Uing brainstem auditory evoked response (BAEP) technique, the authors investigated the changes of BAEP and its monitoring effects, thereby to achieve the aim of reducing the mortality and disable rate of brainstem tumors operation. Methods Five patients with brainstem tumor were monitered by BAEP in each surgical during the entire process of operation. More than 2000 data obtained, including the latency, amplitude and pattern of wave, were analysed and treated statistically with t test and ANOVA.Results All the surgical manipulations, such as stretching, separating, or extruding, might prolong the latency and interval, and diminute the amplitude of wave i, Ill, V; and the differentiation of waves became worse as well. Data revealed that the changes of BAEP might be significantly influenced by the surgical manipulations (P<0.01 ANOVA). There was a marked significance in the difference between BAEP of the ear operative and non operative side (P<0.01 ANOVA). Co"clusion During the operation in part of brainstem and its surrounding areas, BAEP might sensitively reflect the damaged function of brainstem to operators. It's available to monitor these functions with BAEP by monaural stimulation method either in unilateral or bilateral record.
