甲状腺患者围术期睡眠质量现状及其影响因素分析

目的 了解甲状腺患者围手术期睡眠质量的现状,并分析其影响因素,为甲状腺疾病的快速康复指南撰写提供数据支撑。方法 对2016年6月到2016年12月入住本科的689例行甲状腺手术的患者进行入院前及住院期间的睡眠质量对比,并分析导致其住院期间睡眠质量下降的高危因素。结果 甲状腺患者住院期间睡眠总分及各维度得分均高于入院前(P＜0.05)。患者年龄＞49岁、女性、住院天数＞8d、术中行颈淋巴结清扫、未使用助眠药、疼痛评分4-6分及心情评分＞8分是影响患者睡眠质量的高危因素。结论 甲状腺患者住院期间睡眠质量明显低于入院前,其受多种因素的影响。我们应针对导致患者睡眠质量下降的内/外源性因素进行干预,必要时使用助眠药,以加强睡眠管理,保障患者围术期的睡眠质量,加快患者术后康复。

Investigation and analysis of sleep quality and influencing factors in the perioperative thyroid disease patients

Objective To investigate the present situation of perioperative sleep quality of thyroid patients and to analyze its influencing factors to provide data support for the rapid recovery after surgery guideline of thyroid disease. Methods A total of 689 patients who underwent thyroid surgery were enrolled in our hospital during June 10,2016 and December 30,2016.The sleep quality of patients before and after hospitalization was assessed by the Pittsburgh Sleep Quality Index Scale.We compared the sleep quality of the patients before and during hospitalization and analyzed the factors that led to a decline in sleep quality during hospitalization. Results The total scores of sleep and all dimensions of thyroid patients during hospitalization were higher than those before admission (P<0.05).Age>49,female,hospitalization longer than 8 days,neck lymph node dissection,unused sleep aids,pain scores 4-6 and mood scores > 8 were high-risk factors of affecting the patients' sleep quality. Conclusion The quality of sleep during hospitalization for thyroid patients is significantly lower than before admission,which is influenced by many factors.We should intervene in the internal/exogenous factors that lcause the patient's sleep quality to decline,if necessary,use the sleeping medicine to strengthen the management of sleep and guarantee patients sleep quality,and accelerate postoperative recovery.
