摘    要:目的 调查广东省部分健康人群的凝固酶阴性葡萄球菌(CoNS)和耐甲氧西林凝固酶阴性葡萄球菌(MRCoNS)携带情况,并分析MRCoNS菌株的耐药性。方法 对研究对象进行问卷调查和鼻拭子采样,检测MRCoNS菌株的耐药表型和耐药基因,率的比较分析方法采用χ2检验。结果 1 178名研究对象中;CoNS携带率为46.4%;MRCoNS携带率为18.5%。近一个月门诊看病史、下班回家立即洗澡频率对人群MRCoNS携带影响有统计学意义(P均<0.05)。MRCoNS及其亚型对红霉素、四环素、克林霉素、复方新诺明耐药率较高,主要携带四环素耐药基因 tetK 和红霉素耐药基因是 ermC 。结论 健康人群MRCoNS携带率和耐药率均偏高,需加强人群卫生防护工作。

Abstract:Objective To investigate the carriage rates of coagulase-negative staphylococci(CoNS)and methicillin-resistant coagulase-negative staphylococci(MRCoNS)and antibiotic resistance of MRCoNS isolates among some healthy population in Guangdong province. Methods The objects of study were surveyed by face-to-face questionnaire and nasal swab sampling.All MRCoNS isolates were tested for antibiotic-resistant phenotype and genotype.The rates were compared by Chi-squared test. Results Among 1178 study participants,the carrying rates of CoNS and MRCoNS was 46.4% and 18.5%,respectively.The significant influencing factors of human MRCoNS carriage were medical facility visit in the last month and bath frequency after work(P<0.05).The MRCoNS and its subtype isolates were mainly resistant to erythromycin,tetracycline,clindamycin,and trimethoprim- sulfamethoxazole,and mainly carried the tetracycline-resistant gene tetK and erythromycin-resistant gene ermC. Conclusion Both carrying rate and antibiotic-resistant rate of MRCoNS are high in healthy population,so strengthening human health protection is in need.
