摘    要:目的 论证植入心脏再同步治疗(CRT)起搏器前后QRS时限的差值是否能够预测CRT治疗的无反应和超反应.方法 将接受CRT治疗的55例患者,分为无反应组、不包括超反应者的有反应组和超反应组3组,通过组间比较和绘制ROC曲线,论证植入CRT前后QRS时限的差值是否能够预测CRT治疗的反应性.结果 植入后QRS时限在无反应组、不包括超反应者的有反应组以及超反应组依次降低,QRS时限的差值在无反应组、不包括超反应者的有反应组以及超反应组依次升高,只是差异无统计学意义.受试者工作曲线(ROC曲线)显示QRS时限差值不能有效预测CRT治疗的无反应和超反应.结论 QRS时限差值不足以预测CRT治疗的无反应和超反应.

Abstract:Objective To evaluate the predictive value of QRS duration change immediately after implantation for non-response and super-response to cardiac resynchronization therapy. Methods A total of 55 patients who received cardiac resynchronization therapy were divided into non-response group, response group exclusive of super-responders and super-response group. Comparisons between groups and Receiver Operative Characteristic (ROC) curve were employed to determine whether QRS duration change immediately after implantation was able to predict non-response and super-response. Results Post-implantation QRS duration was longest in non-response group, while, successively, in response group exclusive of super-responders and superresponse group. And QRS duration change immediately after implantation increased in the opposite direction compared to post-implantation QRS duration. However, neither of the above parameters had significant statistic difference among the three groups. Furthermore, ROC curve showed that QRS duration change could not predict non-response and super-response to cardiac resynchronization therapy. Conclusion QRS duration change immediately after implantation is inadequate to predict non-response and super-response to cardiac resynchronization therapy.
