巨噬细胞炎症蛋白-2在急性胰腺炎大鼠肺损伤中的作用

摘    要:目的:研究巨噬细胞炎症蛋白-2(MIP-2)与急性胰腺炎(acute pancreatitis,AP)大鼠肺损伤的关系。方法:雄性SD大鼠,随机分为5组,即正常对照组,AP3 h,AP6 h,AP12 h,AP24 h组,胰胆管逆行注射脱氧胆酸钠,制备大鼠急性胰腺炎模型。测定血清淀粉酶,测定肺组织MIP-2含量和髓过氧化物酶(MPO)活性,胰腺组织和肺组织病理学检查及评分。结果:AP 3 h组肺组织中MIP-2的含量即明显升高,6 h达到高峰,和正常对照组比较差异有统计学意义;肺组织中MPO活力3 h开始升高,并随时间推移而逐渐升高;AP 3 h组肺组织及胰腺组织病理学评分明显高于正常对照组,在6 h及以后时段损伤表现更为严重。本研究结果中肺镜下病理评分与胰腺镜下病理评分(r=0.35,P<0.05)、肺组织MPO活性(r=0.42,P<0.05)呈正相关。结论:急性胰腺炎病理过程中,MIP-2作为早期的促炎性细胞因子,趋化和激活了以中性粒细胞为主的炎性细胞,介导了肺损伤。

Abstract:Objective:To investigate the changes of macrophage inflammatory protein 2(MIP-2)in lung injury induced by acute pancreatitis(AP).Methods: SD rats were divided at random into five groups:AP 3 h group,AP 6 h group,AP 12 h group,AP 24 h group and control group,Rat AP model was induced by intraductal administration of 5% sodium deoxycholate(DCA).Animals were sacrificed at 3,6,12,24 hours after induction of pancreatitis.The following parameters were measured:Serum amylase,the intrapulmonary content of MIP-2,myeloperoxidase(MPO) activity,histopathological scoring of lung and pancreatic tissues. Results: The intrapulmonary content of MIP-2 increased significantly in AP 3 h group,with peaking around 6 hours.Myeloperoxidase(MPO) activity overpressed in AP 3 h group and persisted up to 12~24 hours.Histopathological Score of lung tissues correlated well with Histopathological Score of pancreatic tissues(r=0.35,P<0.05)and intrapulmonary MPO(r=0.42,P<0.05). Conclusion: MIP-2 was highly correlated with lung injury induced by AP,may induce pulmonary leucocyte infiltration and activation in the early stage.
