贵阳城区老年慢性病人健康状况及社区卫生服务需求调查

目的了解贵阳市城区老年慢性病人患病情况,卫生服务需求情况及影响因素。方法本研究采用分层整群随机抽样调查方法 ,选取贵阳城区1185名老年人,通过采用日常生活能力量表（ADL）、生存质量量表（WHOQOLBREF）、社会支持评定量表（SSRS）、自编的卫生服务需求问卷,同时结合60岁及以上老年人建立的健康档案进行调查,了解老年慢性病人的一般情况、疾病史、日常生活能力、生存质量及社区卫生服务需求情况。结果 （1）贵阳部分城区老年人慢性病患病率为50.5%,2周患病率为334.171‰;（2）日常生活能力有不同程度受损的老年慢性病人占27.9%,其中19.1%的老年慢性病人为轻度依赖（仅有IADL功能受损）,中、重度依赖者占8.8%;（3）慢性病是影响老年人生命质量的首要因素;（4）有10.4%的老年慢性病人认为不需要社区卫生服务。在选择需要的社区服务项目中,仅定期体检所占比例较高（58.4%）,实际常到社区卫生中心就诊的老年慢性病人仅20.3%。结论老年慢性病人的卫生服务客观需要量大,但社区卫生服务需求意愿较低,客观需要与主观需求及实际利用脱节。

A survey on the health status and the requirement of community health service of senile chronic patient in Guiyang city

Objective To know the sickness condition of senile chronic patient,the requirement of health service of senile chronic patient and influencing factor. Methods Stratified sampling was adopted and 1185 elderly people in GuiYang city were investigated,adopt activity of daily living scale,WHO Quality of life-BREF,Social supp-assessment shect,questionnaire on community health service and the health record of the people who were 60 or over 60 years old to conduct. To know general conditions,the history of disease,activity of daily living,quality of life and the requirement of community health service.Results （1）The morbidity rate of chronic diseases of the aged in GuiYang city is 50.5 %.Two-week morbidity rate is 334.171%o. （2）The people whose activity of daily living was damaged in various degrees accounted for 27.9 %,mild dependences accounted for 19.1% inside,moderrate or severe dependences accounted for 8.8%. （3）chronic diseases is the vital factor which can influence the aged＇ s quality of life. （4） 10.4% of senile chronic patient thought that who didn＇t need community health service. Only regular physical examination accounted for a litter large proportion in these choosed items（58.4%）.Actually,thepatientswhoused to go toCHC is only 20.3%.Conclusion The objective requirement of health service of senile chronic patient is high,but the subjective desire to community health service is low. objective requirement,subjective desire and actual use ungear each other.
