腹腔镜治疗肾囊肿16例

目的 探讨腹腔镜手术治疗单纯性肾囊肿疗效。方法 采用经腹腔和经腹膜后腹腔镜技术对16例单纯性肾囊肿行囊肿去顶术,并回顾分析比较20例开放性肾囊肿去顶术。结果 经腹腔途径手术时间与开放手术无显著性差异(P>0.05),经腹膜后途径手术时间显著长于开放手术(P<0.05),腹腔镜手术患者的术中失血量、术后并发症及术后住院时间均显著少于开放手术(P<0.01)。结论 腹腔镜肾囊肿去顶术具有创伤小、术后并发症少及康复快等优点,治疗单纯性肾囊肿的疗效明显优于开放性手术。

Treatment of 16 patients with simple renal cysts with laparoscopic surgery

AIM To evaluate the effect of laparoscopic unroofing of simple renal cysts and to compare the result to hat of open surgery group. METHODS By using transperitoneal laparoscopy and retroperitoneal laparoscopy, renal cyst unroofing of 16 cases was performed. Among the 16 cases , 11 were performed through transperitoneal approach, and in the other, renal cyst unroofing was performed through retroperitoneal approach. Open renal cyst unroofing in 20 cases were retrospectively analyzed. The effect of treatment was compared between two groups. RESULTS Compared with open surgical group , operation time of transperitoneal laparoscopic renal cyst unroofing had no significant difference (P>0.05). Operation time of retroperitoneal laparoscopic renal cyst unroofing was significantly longer (P<0.05). But laparoscopic group had significant less blood loss and fewer complications and shorter hospital stay than open surgical group(P<0.01). CONCLUSION The transperitoneal or retroperitoneal laparoscopic renal cyst unroofing has the advantages of less trauma and fewer complications and rapid recovery in treating simple cyst. It is superior to those open renal cyst unroofing in respect of the therapeutic effects.
