肾镜直接扩张通道在经皮肾镜取石术的应用

目的研究肾镜直接扩张通道在经皮肾镜取石术中的应用价值。方法对80例肾结石及输尿管上段结石患者随机分为2组,对照组40例,51侧,用筋膜扩张器逐一扩张工作通道。实验组40例,55侧,用肾镜在造瘘管的引导下直接扩张工作通道进入肾盂。结果对照组用筋膜扩张器扩张通道发生4侧通道丢失,5侧发生肾脏对穿损伤,通道扩张成功率92.2%,肾脏对穿损伤率9.82%;实验组用肾镜直接扩张通道无一侧发生通道丢失,无一侧发生肾脏对穿,通道扩张成功率100%,肾脏对穿损伤率0%;实验组通道扩张成功率明显高于对照组,肾脏对穿损伤率明显低于对照组,两组比较P<0.05,差异有统计学意义。结论用肾镜直接扩张通道在通道丢失及肾脏对穿损伤方面均具有较高的安全性。

Percutaneous pyeloscopic expansion of channel in nephrolithotomy

Objective To research pyeloscopic direct expansion channel in nephrolithotomy.Methods 80 patients with renal and ureteral callus were randomly divided into two groups.Control group（40 cases,51 sides）undertook the fascia to expand work channel.Experimental group（40 cases,55 sides）undertook pyeloscopic expansion.Results In the control group,4 sides channel lost,5 sides kidney damaged and the channel expansion rate was 92.2%.In experimental group,no channel lost,no renal damage occurred,and the channel expansion rate was 100%.The success rate was significantly higher than that of the control group.Conclusion Pyeloscopic direct expansion is successful and safe.
