B型超声在腹内脏器损伤诊断中的应用

目的 探讨腹内脏器损伤的声像图特点以及超声诊断价值。方法 应用二维超声对１８０例腹内脏器损伤患者进行超声检查，并与手术及临床诊断结果对照。结果 １８０例患者中裨性脏器损伤９６例，空腔脏器损伤３１例，脏器损伤伴腹腔积液１３９例，占７７．２％，腹膜后血肿２８例。Ｂ超诊断完全符合占６７．２％（１２１／１８０），部分符合占１５％（４５／１８０），不符合占７．８％（１４／１８０）。结论 腹内脏器损伤的声像图表现与损伤部分、程度和时间密切相关、腹腔积液是判断有无脏器损伤最敏感的指标。据此超声能对脏器损伤分型和程度判断，能为手术抉择提供重要依据。

Application of B - type ultrasound in the diagnosis of injury in abdominal organs

bjective: To discuss the features of sonography and significance of ultrasound diagnosis in injuries in abdominal organs. Method: Two - dimensional ultrasound was cpplied in the ultrasound examination over 180 cases of patients of injuries in abdominal organs and making contraposition with surgical and clinical diagnosis, Result: Among the 180 cases, 96 were virtual injury of abdominal organs, 31 were injury of hollow viscus, 139 were injury of abdominal organs with hydrops abdominis, which took up 77. 2% of all, and the other 28 cases were post abdominal cavity hematoma. A proportion of 67. 2% of the B - type ultrasound diagnoses were accordant (121/180), 15% were partly accordant(45/180) and, 7.8% were inaccordant (14/180) . Conclusion: Sonography of abdominal organ injuries is closely related to the part injured, degree as well as period of injury, and hydropsabdominis is the most sensitive index in the judgement of abdominal organ injuries. Accordingly, the result of B - type ultrasound facilitates classification of types as well as judgement over the degree, of injury of abdominal organs, and provides significant evidences for the decisions on surgeries.
