电视胸腔镜手术在胸部疾病诊断和治疗中应用的体会

目的　探讨电视胸腔镜手术 (VATS )在胸部疾病诊断和治疗中的作用。方法　 1996年 9月以来选择性地开展了电视胸腔镜手术 6 6例 ,其中肺大疱及自发性气胸 32例、肺结核球和结核性空洞 14例、肺恶性肿瘤 5例、肺良性肿瘤 5例、肺包虫囊肿 2例及纵隔肿瘤 3例 ,肺动静脉瘘、胸膜脂肪瘤、恶性胸膜间皮瘤、局限性结核性脓胸、外伤性膈疝各 1例。结果　VATS手术创伤小、失血量少、手术时间短、病人痛苦轻、恢复快、缩短住院日、无严重并发症。结论　VATS诊断和治疗胸部部分疾病是一种安全、有效、微创的手术方法。

Experiences of Diagnosis and Treatment in Chest Disease with Video-Assisted Thoracoscopic Surgery

Objective To evaluate the feasibility of videoassisted thoracoscopic surgery(VATS) in the diagnosis and treatment of chest disease. Methods Sixtyfive selected cases were performed VATS during September 1996 to February 2003. Among these cases there were spontaneous pneumothorax and pneumatocele(n=32) cases, tuberculoma and tuberculous cavity(n=14), pulmonary malignant tumor(n=5), pulmonary benign tumor(n=5), pulmonary hydatid(n=2) cyst and mediastinum tumor(n=3), pulmonary arteryvein fistula,pleuralipoma,malignantpleura mesothelioma,tuberculous limited empyema, traumatic diaphragmatic hernia(n=1) respectively. Results The injury in VATS was minimal with little blood lose, shortening operation time, mild suffering and rapid recovery. In hospital duration was short without significant postoperative complications.Conclusion Videoassisted thoracoscpic surgery in diagnosis and treatment of selected thoracic diseases is effective, safe and minimal invasive.
