生物型双极人工股骨头置换治疗高龄股骨颈骨折的疗效观察

摘    要:目的探讨应用双极人工股骨头置换治疗老年人股骨颈骨折的疗效。方法回顾性分析1999年3月至2007年7月应用双极人工股骨头置换治疗16例老年股骨颈骨折患者的临床资料。按Garden分型:Ⅱ型1例,Ⅲ型10例,Ⅳ型5例。结果平均手术时间为75min,术中平均出血量为200mL。16例患者均获随访,随访时间l5~36个月,平均22个月;按Harris髋关节功能评分标准:优12例,良2例,可1例,差1例。结论双极人工股骨头置换术是治疗高龄老年股骨颈骨折的良好方法。

Abstract:Objective To investigate the clinical outcome of hemiarthroplasty treated femoral neck fracture in elder people. Methods From March 2000 to July 2007,16 elderly patients of over seventy--five years old with femoral neck fracture were operated with bipolar prosthetic replacement. There were 2 males and 14 females,with the average age of 76.5 years old, The fracture type was Garden Ⅱ, Ⅲ or Ⅳ, and all the patients underwent the hip hemiarthroplasty of domestic prosthesis, were followed up for 15 months to 21 months with an average of 22 months. The Harris's hip score system after operation was adopted for the follow--up. Results Of the 16 patients, the mean time for operation was 75 minutes, the mean blood loss was 200ml. All of the patients were followed up for 15 months to 21 months with an average of 22 months. According to the Harris's scoring system, there were 18 excellent or good(87.5%)in bipolar prosthetic replacement group. Conclusion The bipolar prosthetic replacement are reasonable choice for treatment of fracture of femoral neck in the elderly.
