经尿道前列腺电汽化术前后血清前列腺特异性抗原值的变化及意义

目的 :研究良性前列腺增生患者行经尿道前列腺电汽化术 ( TUEVP)对前列腺特异性抗原 ( PSA)值的影响。方法 :经直肠超声测量 3 0例前列腺增生患者的前列腺体积 ,均行 TUEVP。分别于术前及术后第 1、3、5天及 1个月检测患者血清中 PSA值。并用中位数±四分位间距成组设计的秩和检验进行显著性检验。结果 :术后第 1天血清 PSA值较术前明显升高 ( P<0 .0 0 1) ,其后逐渐下降 ,至术后 1个月血清 PSA值降至基线水平 ,与术前比较无显著性差异 ( P>0 .0 5 )。结论 :TUEVP会使血清 PSA值升高 ,以术后第 1天影响最明显 ,维持约 1个月 ,其后逐渐下降至一个稳定、较低的水平。所以 ,临床上如要获得一个正确而有意义的 PSA值 ,至少要等到 TU EVP术后 1个月。

The Significance of Serum Prostate Specific Antigen after Transurethral Electro-vaporization of the Prostate

Objective:To assess the effect of transurethr al elect rovaporization of the prostate(TUEVP) on serum prostate specific antigen(PSA)con centration.Methods:Thirty men aged 64.5 years(59~75)with symptoms of prostatism un derwent TUEVP. Serum PSA levels were determined before any prostatic manipulatio n and again 1,3,5 day and 1 month after TUEVP. Prostate volume was measured by t ransrectal ultrasonography before TUEVP.Results:The mean serum PSA concentration was significantly higher at 1 day after TUEVP( P <0.001) than before, and then continuous decline. The PSA level returned to baseline 1 month after TUEVP( P >0.05). Conclusion:TUEVP increases serum PSA levels, the transient elevation p ersisting for up to 1 month then declining to a stable?low PSA concentration. T herefore, it is important to wait at least 1 month to obtain an accurate and mea ningful serum PSA level after TUEVP.
