院内感染后败血症

摘    要:本文报道院内感染后败血症97倒,均经血培养证实。致病菌113株,其中革兰阳性球菌30株(26.5%),革兰阴性杆菌76株(67.2%),真菌5株(4.4%),厌氧菌2株(1.8%)。致病菌中以大肠杆菌(25株)为最常见,其次是肺炎杆菌、绿脓杆菌、金黄色葡萄球菌,表皮葡萄球菌、凝聚肠杆菌及四联球菌。单菌种败血症82例、复数菌败血症15例。所有病例均有严重基础性疾病,起病急骤,昏迷15例,感染性休克64例。62例死亡,病死率63.9%。

Abstract:97 documented cases of nosocomlal septicemia were encountered from 1980 to 1986. Altogether 113 strains of pathogenic organisms were obtained in these 97 cases. 30 strains (26.6%) were Gram-positive cocci, 76 strains (67.2%) Gram-negative bacilli, 5 strains (4.4%) fungi, 2 strains (1.8%) anaerobic bacteria.In the 113 strains of pathogenic organism, Escherichia coli (25 strains) were the most frequent pathogens, followed by Klebsiella pneumoniae, Pseudomonas aeru-ginosa, Staphylococcus aureus, Staphylococcus epidermidis, enterobacter agglomerans and Tetracoccus.In the 97 cases, 82 (84.5%) were single pathogen septicemias and 15 cases (15.5%) were caused by multiple pathogens.All oases had severe underlying disease, which included pneumonia, pyogenic cholecystitis, acute leukemia, lymphoma, malignant histiooytosis, acute hepatic failure, hepatic cirrhosis, drug dermatitis, collagen-vascular disease, diabetes, myelitis, etc.Onset of the disease was usually abrupt. The patients were usually seriously ill. Coma occurred in 15 of the 97 cases, septic shook developed in 64 cases.62 of the patients died, a mortality was 63.9%. Deaths of 62 patients were caused by septic shook, acute renal failure, intraoranial hemorrhage, acute hepatic failure and DIC.
