静脉注射胺碘酮治疗老年充血性心力衰竭伴房颤

目的观察静脉注射胺碘酮治疗老年充血性心力衰竭并发心房颤动的有效性及安全性。方法对56例充血性心力衰竭伴房颤患者,给予胺碘酮150mg,15～20min后无效再给予75mg,之后以0．5～1．0mg/min静脉滴注24h(24h总量＜1200mg)。结果平均负荷量为150～300(170±38)mg,38例转为窦性心律,转复率67．9%,18例仍为房颤,但心室率下降为100次/min以下,不良反应发生率10．7%,无1例心功能恶化。结论静脉注射胺碘酮治疗老年充血性心力衰竭并心房颤动是有效和安全的。

A study on the efficacy and safety of intravenous amiodarone in the treatment of senile patients with congestive heart failure complicated with atrial fibrillation

Objective To evaluate the efficacy and the safety of intravenous amiodarone in the treatment of senile patients with congestive heart failure complicated with atrial fibrillation.Methods The 56 patients with this diseases were included in this study.The first bolus of amiodarone was given at 150mg and if the result was not satisfactory 15-20minutes after the bolus injection half the bolus were again given followed by sustained at the speed of 0.5-1mg/min in 24 hours.Results The average loading dose was 150-300 (170±38)mg,38 patients returned to sinus rhythms (67.9%),18 patients with continous atrial fibrillation after amiodarone infusion had ventricular rate below 100 bpm.Adverse reaction were observed in those patients (account for 10.7%).Conclusion Amiodarone is safe and effective in management of atrial fibrillation associated with congestive heart failure in senile patients.
