肩胛骨骨折的分类及治疗方法的选择

目的 探讨肩胛骨骨折（SF）的分类，评价其对临床治疗方式选择的指导作用．、方法对38例虾患者行X线检查，其中螺旋CT三维苇建检查12例。根据Hardegger和Miller的分型方法分别采用保守治疗（20例）和手术治疗（18例）。结果38例均获随访，时间9～50个月。根据Rowe的疗效评价标准：非手术组：优14例，良3例，可1例，差2例；手术组：优12例，良2例，可3例，差1例。结论多数盯尢需手术治疗，经短期制动及功能锻炼后即可获得满意疗效，但移位严晕的SF及飘浮肩损伤等应手术内固定。

Classification and treatment of scapular fractures

Objective To explore the classification and to evaluate its guidance for the clinical treatment of scapular fractures （ SF）. Methods Thirty-eight patients with SF were retrospectively reviewed. All of them were examined by conventional radiography, and 12 were also examined by three-dimensional spiral CT reconstruction. According to Hardegger and Miller＇s classification for SF, 20 patients received conservative treatments and 18 surgical treatments. Results All cases were followed up lot 9 - 50 months. According to Rowe＇s evaluation criterion of therapeutic efficiency, 14 were excellent,3 good, 1 fair, and 2 poor with non-surgical conservative treatments and 12 excellent, 2 good,3 fair,and I poor with surgical treatments. Conclusions The majority of SF needn＇t operation. The satisfactory therapeutic efficiency can be achieved by short-term immobilization and functional training. But for the patients with severe displacement and floating shoulder injury etc, open reduction and internal fixation are required. Open reduction and internal fixation are the useful and safe technique for the treatment of specific displaced fractures of scapula.
