骨盆骨折DR摄影与CT检查的临床应用价值

目的探讨DR摄影与CT检查技术在常见复杂骨盆骨折诊断中的临床应用价值。方法对64例骨盆外伤患者行常规DR摄影，部分病例再行螺旋CT扫描分析。结果13例显示阴性，27例简单型骨折的诊断准确率为87％，21例复杂型骨盆及髋臼的诊断准确率为73．6％，3例股骨颈骨折漏诊；CT对简单和复杂型骨盆及髋臼骨折的诊断准确率为100％，9处关节腔内碎骨片及骶椎隐匿性骨折均能显示，而DR仅显示3处。结论DR摄影作为骨盆骨折常规检查，弥补了普通x线摄影的不足，提高了摄片利用率和更高的影像质量，与CT检查及后处理技术互补，可以根据需要从不同的方位显示损伤情况，提高了诊断的正确率，具有较大的临床应用价值。

Clinical Value of DR and CT Examination for Pelvic Fracture

Objective To explore the clinical value of DR and CT examination in complex pelvic fracture diag- nosis. Methods 64 patients with pelvic trauma underwent conventional DR, some of whom underwent spiral CT exam- ination afterwards. Results The results of 13 patients were negative. Diagnostic accuracy was 87% for 27 patients with simple fracture and 73.6% for 21 patients with complex pelvic and acetabular fracture. 3 patients with femoral neck fracture were missed diagnosis;Diagnostic accuracy of CT examination was 100% for simple and complex pelvic fracture as well as acetabular fracture. 9 intra - articular bone fragments and sacral vertebral occult fractures could be shown by CT examination,while 3 could be shown by DR. Conclusion DR as a routine examination for pelvic frac- ture can make up the shortage of ordinary x - ray photography,improve utilization rate of radiography and raise image quality. DR and CT are complementary, which can show injury tYom different directions as needed and improve diag- nostic accuracy rate.
