结直肠脂肪瘤20例诊治分析

目的：回顾总结20例结直肠脂肪瘤的诊断与治疗的经验。方法：分析自1993年—2007年治疗的20例结直肠脂肪瘤患者的临床资料。结果：结直肠脂肪瘤的临床表现无特异性,可并发肠套叠或肠梗阻,超声内镜的诊断准确率为93.8%。本组病例中9例行内镜下脂肪瘤切除术,11例行开腹手术（局部切除术或肠部分切除、吻合术）,17例（85.0%）随访1~168个月,均无复发或转移。结论：超声内镜是诊断胃肠道脂肪瘤的有效方法,开腹手术切除或内镜下手术切除脂肪瘤是有效的治疗手段,内镜下手术是结直肠脂肪瘤微创治疗的发展方向。

Diagnosis and Treatment of Colorectal Lipomas: a Report of 20 Cases

Objective：To summarize the experience of diagnosis and treatment of colorectal lipomas.Methods：The clinical data of 20 patients with Colorectal lipomas were analyzed retrospectively from 1993 to 2007.Results： Colorectal lipomas can produce a variety of symptoms including Colorectal bleeding,abdominal pain,anemia,intussusception,and bowel obstruction.The diagnostic accuracy of endoscopic ultrasonography（EUS） for Colorectal lipomas was 90.0%.Nine GI lipomas were resected by Endoscopyic Mucosa Resection（EMR） and Endoscopyic Submucosa Dissection in the study,the reminder at laparotomy,partial resection,or enterectomy being performed.17 cases were followed up from 1 month to 168 months,no recurrence and metastasis were observed.Conclusions：EUS is an effective method for diagnosing Colorectal lipomas.The surgical treatment of Colorectal lipomas is accepted,however,endoscopic removal of Colorectal lipomas is also a safe way.
