医源性脾损伤的防治对策

目的：分析医源性脾损伤的原因,并探讨其预防和治疗对策。方法：对1984年7月—2009年7月21例与手术有关的脾损伤患者的临床资料进行回顾分析。结果：21例脾损伤发生于胃手术9例（42.8%）,结肠手术6例（28.6%）,肝脏手术3例（14.3%）,其他手术3例（14.3%）。各例均经手术治愈,其中行脾切除术12例（57.1%）,脾修补术4例（19.1%）,局部止血5例（23.8%）。结论：医源性脾损伤大多数是可以预防的,根据脾损伤的程度选择合理术式是治疗的关键。

Prevention and Treatment of Iatrogenic Splenic Injuries

Objective：To analyse the causes of iatrogenic splenic injuries and investigate the measures of prevention and treatment.Methods：The data of 21 patients with splenic injuries caused by operations from July 1984 to July 2009 in our hospital were analysed retrospectively.Results：The major causes included undergoing operations on stomach（9/21,42.8%）,colon（6/21,28.6%） and livers（3/21,14.3%）.The operation procedures for these cases were simple splenectomy（12/21,57.1%）,simple repair of spleen（4/21,19.1%） and local hemostasis（5/21,23.8%） respectively.Conclusions：Most of iatrogenic splenic injuries caused by operations can be prevented.It is very important to choose the correct surgical method under different circumstances of iatrogenic splenic injuries.
