2种不同方式放置冠状静脉窦电极效果的比较

目的：观察穿剌锁骨下静脉和穿刺颈内静脉的2种不同方式放置冠状静脉窦电极的有效性和安全性。方法：293例心动过速患者分为2组;（1）穿刺右颈内静脉放置冠状窦电极组,（2）穿刺锁骨下静脉放置冠状窦电极组。每一种方法如穿刺5次未成功则改为另一种方法。观察2组放置冠状窦电极的成功率和并发症情况。结果：右颈内静脉组145例,穿刺成功142例,不成功3例,无并发症出现。左锁骨下静脉组148例,穿刺成功140例,不成功8例,2例出现气胸,1例出现血气胸,1例因压缩面积大于30%行胸腔穿刺抽气,另2例未行特殊处理自行吸收,3例均未留下后遗症。2组中不成功改为另一穿刺方式进行穿刺均获得成功。2组放置冠状静脉窦的时间无差别。结论：2种方式放置冠状窦电极均成功率高,但经颈内静脉放置冠状静脉窦电极具有较低的并发症发生率。

The Comparison the Effect of Placing Coronary Vein Sinus Catheter by Two Different Type of Pathway

Objective：To study the efficacy and safety of placing coronary vein sinus catheter by two different type of pathway.Methods：A total of 293 patients with tachycardia acept radiofrepuency ablation were divided into two groups.One group was given placing coronary vein sinus catheter by righ internal jugular vein puncturation,another group was given placing coronary vein sinus catheter by subclavian vein puncturation.If one type can not process successfully,another type was used.The successful rate and safety were evaluated.Results：In group with righ internal jugular vein puncturation,there were 142 of 145 successful,no complication occurred.In group with subclavian vein puncturation,140 of 148 were successful,pneumothorax occurred in 2 cases,the process time in two type were not different.Conclusions：There has a high successful rate in two different type of pathway placing coronary vein sinus catheter,but coronary vein sinus catheter placement by righ internal jugular vein pathway has less complication than subclavian vein pathway.
