老年高血压患者血压晨峰与心率变异性的相关性研究

目的观察老年高血压患者血压晨峰与心率变异性(HRV)的关系及临床意义。方法按血压晨峰定量标准将342例老年高血压患者分为晨峰组与非晨峰组,并选取50例健康人作为正常对照组。比较各组HRV参数。结果三组HRV各参数的比较差异有统计学意义。晨峰组24h内全部正常心动周期的标准差、5min正常R-R间期均值的标准差、24h相邻正常NN间期差值均方根、相邻正常R-R间期差值>50ms的个数占心跳总数百分比以及心率变异指数较非晨峰组显著降低,差异有统计学意义(P<0.01)。结论老年高血压患者血压晨峰与HRV有关,晨峰程度越大,HRV参数降低越明显,提示患者有自主神经功能的损害。

Study of the Relevance between the Morning Blood Pressure Surge in Elderly Patients with Hypertension and the Heart Rate Variability

Objective To observe the relation between the morning blood pressure surge in elderly hypertensive patients and the heart rate variability( HRV) and its clinical significance. Methods 342 cases of elderly hypertensive patients were divided into the morning blood pressure surge groupand the non-morning blood pressure surge group accdording to quantitatiion standard of morning blood pressure surge. And 50 healthy persons were selected as normal control. The parameter of HRV in each group was assessed. Results The HRV parameters of three groups had statistically significant difference. The results of the morning peak the standard deviation of 24h all normal cardiac cycle,the mean standard deviation of 5min normal R-R interval( SDANN) ,24h normal interval adjacent NN RMS difference,the number of normal adjacent RR interval difference greater than 50ms percentage representing the total number of heartbeats,and the heart rate variability index in the group with the morning blood pressure surge was significantly reduced compared with the group without the morning blood pressure surge,which shows a statistical significance( P < 0. 05) . Conclusion The morning blood pressure surge in elderly patients with hypertension was related to HRV. HRV parameters decreased more obviously with the greater level of morning blood pressure surge,it suggested that autonomic neuropathy of patients to be damaged.
