胸心血管外科ICU患者转运交接单的设计与应用

目的　提高胸心血管外科患者 ICU 转出交接班质量，避免护理差错和纠纷。 方法　2010 年 1 月－6 月，针对患者转运出现的问题，在查阅文献及专家咨询的情况下，自行设计胸心血管外科 ICU 患者转出交接单，对患者转出 ICU 时的病情、用药、并发症等信息进行完善，同时规范交接班签字手续。尔后采用调查问卷方式，对交接单运用效果进行评价。 结果　转出交接单的应用，提高了病房护士对 ICU 护士交班质量满意度，其增加率波动在 0.00% ～ 33.4%。 结论　转出交接单的使用能够提高 ICU 患者术后转运交接质量，预防护理差错和纠纷，确保证患者安全。

The Designing and Application of Transporting Handover Form for Patients in ICU of Cardiovascular Surgery Department

Objective To improve the handover procedure for patients intended to be transferred out from the ICUof thoracic and cardiovascular surgery department, in order to avoid nursing errors and clinical disputes. Methods　FromJanuary to June 2010, with the help from the literature and specialized experts, we designed handover form by ourselves fortransferring patients out from the ICU of the thoracic and cardiovascular department, in order to avoid problems in patienthandover between various departments. The handover form standardized patients’ conditions, drug use, complicatingdiseases and other information. Simultaneously, whoever did the handover was demanded to sign the form. The effects ofthe form were assessed by means of questionnaire. Results　The application of such handover form increased ward nurses’satisfaction degree concerning the handover quality of ICU nurses, which fluctuated within 0-33.4%. Conclusion　Theapplication of handover form can improve the handover quality of ICU patients after operation and prevent nursing errorsand disputes, both of which can ensure patients’ safety.
