寻常性银屑病患者外周血Th1和Th17细胞亚群的研究

目的检测寻常性银屑病患者外周血中Th极化细胞Th1和Th17细胞数量和比例的变化,探讨其在银屑病发病机制中的作用。方法选择30例寻常性银屑病患者,采用流式细胞术(FCM)检测寻常性银屑病患者外周血中CD4+T细胞上IFN-γ+IL-17-(Th1)、IL-17+IFN-γ-(Th17)的表达,并与健康对照组比较,分析寻常性银屑病严重指数PASI和Th极化细胞表达量、Th1/Th17比值之间的关系。结果寻常性银屑病患者PBMC中IFN-γ+Th1细胞的数量、IL-17+Th17细胞的数量均高于健康对照组(P<0.01);Th1/Th17比值低于健康对照组(P<0.01),Th17细胞的数量与PASI呈正相关(P<0.01,r=0.726),Th1/Th17细胞比值与PASI指数呈负相关(P<0.01,r=0.573)。结论寻常性银屑病患者外周血存在Th1/Th17细胞失衡现象,Th1/Th17比值变化可能参与银屑病患者的免疫学发病机制。

Detection of quantity of Th1 and Th17 subsets in peripheral blood of the patients with psoriasis vulgaris

Objective To analyze the expressions of Th-polarized cells of Th1 and Th17 in peripheral blood in patients with psoriasis vulgaris.Methods Expressions of IFN-γ+IL-17-(Th1),IL-17+IFN-γ-(Th17) on CD4 T cells in peripheral blood in patients with psoriasis vulgaris were detected by flow cytometry,which were compared to those in healthy controls.The correlation was analyzed between PASI score and percentage of Th-polarized cells and Th1/Th17 ratio.Results The mean percentage of Th1,Th17 cells in peripheral blood in psoriasis vulgaris patients was significantly different from that in healthy controls(P< 0.01).The Th1 cells and Th1/Th17 ratio in psoriasis vulgaris patients were correlated with PASI score(P<0.01).Conclusion The predominance of Th17 in peripheral blood of psoriasis vulgaris and Th1-dominated immunity may play an important role in the pathogenesis of psoriasis vulgaris.
