牵张成骨垂直升高牙槽骨的临床应用

目的:探讨牵张成骨垂直升高牙槽骨的可行性和临床效果。方法:对4例因外伤造成牙槽骨垂直向缺损,且缺损均>10 mm的患者采用垂直向牵张成骨术。其中男性3例、女性1例,年龄31～46岁。安放牵张器7 d后开始牵张,每天牵张0.9 mm,直到要求的高度。保留牵张器3个月后拆除牵张器。常规行术前、术后、牵张结束后,及拆除牵张器前X线检查。结果:4例患者均牵张成功,达到预定的高度,无一例感染,有一例成骨偏向舌侧。结论:利用垂直牵张成骨增加牙槽骨高度是治疗牙槽骨垂直缺损的可行方法。

Clinical Application of Distraction Osteogenesis in Vertical Alveolar Ridge Augmentation

Objective To explore the way of obtaining bony height of the mandibular alveolar ridge by vertical distraction osteogenesis.Methods: 4 patients with vertical alveolar ridge deficiencies caused by trauma were treated with vertical distraction osteogenesis.Three patients were male and one was female,whose ages were in 31～46 years old.Regular radiographic examination was done preoperatively,post-operatively,and distraction completed.Vertical distraction of alveolar bone was started one week after the distractor was inserted.The height of distraction was 0.9 mm per day till enough height was obtained,then the distractor was removed three months later.Results: All 4 cases gained successful and enough alveolar height.No patient was infected.Regenerative alveolar tended to grow to lingual aspect in one case.Conclusion: Vertical distraction osteogenesis is a feasible method in treating vertical alveolar ridge deficiencies.
