中年阻塞性睡眠呼吸暂停综合征与脑白质损害关系及护理干预的研究

目的探讨中年阻塞性睡眠呼吸暂停综合征(OSAHS)与脑白质损害(WML)的关系及护理干预的疗效。方法收集45例中年阻塞性睡眠呼吸暂停综合征患者及50例单纯肥胖者,所有入组者完成神经心理学评估,头颅MRI检查,护理干预。结果与单纯肥胖组比较,两组间年龄、性别、受教育程度、BMI无明显统计学差异(P>0.05),而AHI,平均SaO2和平均最低SaO2有显著统计学差异(P<0.05);与单纯肥胖组比较,OSAHS患者脑白质损害较单纯肥胖者明显,差异有统计学意义(P<0.05);OSAHS患者随着病情加重,WML越严重,差异有统计学意义(P<0.05);与单纯肥胖组比较,OSAHS患者总体认知功能无显著差异(P>0.05),而其记忆认知领域损害明显统计学差异(P<0.05);Logistic回归分析显示,OSAHS患者WML与AHI、平均SaO2密切相关;护理干预前后,OSAHS患者睡眠呼吸指标较前明显好转。结论 OSAHS会导致脑白质损害,从而引起记忆功能障碍。护理干预后OSAHS患者睡眠呼吸指标明显好转。因此,尽早对OSAHS患者采取干预措施,有利于减缓脑白质损害及认知功能障碍。
