24小时动态心电图与平板运动试验比较对冠心病的诊断价值

本文对166例冠心病患者进行24小时动态心电图分析,结果表明,动态心电图检出心律失常308例次(93.9%)而常规心电图检出34例次(20.4%),二者有显著差别(P<0.01)。166例动态心电图检出 ST-T 阳性64例(39.1%),常规心电图检出 ST-T 阳性5例(3%)。其中41例冠心病动态心电图与平板运动试验对比,两项结果一致者25例(60.9%)。11例平板运动试验阳性而动态心电图阴性,5例动态心电图阳性而平板运动试验阴性,两者同时进行可提高冠心病的检出率。动态心电图能检出有心绞痛发作和无症状患者的 ST-T 改变,检查方法简便,有较高的安全性值得推广。

Diagnostic Value of Ambulatory Monitoring EKG As Compared with Treadmill Exercise Test in Coronary Heart Disease

The results of 24 hour ambulatory monitoring electrocardiography in 166 patientswith coronary heart disease were analysed.308 episodes(93.9%)of arrhythmiawere detected,while only 34 episodes(20.4%)were discovered in routine 12-leadelectrocardiography(P<0.01).64 patients(39.1%)were demonstrated positive ST-T,in contrast with 5 positive ST-T(3%)in routine electrocardiography.41 of the 166patients were compared with treadmill exercise testing.25 patients(60.9%)had theidentical results,eleven patients had the positive results in treadmill exercise testingbut negative in ambulatory electrocardiography.5 patients were positive in ambulatorybut negative in treadmill exercise testing.By combining both methods,more dpportu-nities to diagnose coronary heart disease could be attained.With ambulatory elec-trocardiography,the onset of angina pectoris and ST-T changes without symptomscould be detected.This method is worth recommended owing to its simplicity andsafety.
