鸦胆子油乳剂联合白细胞介素-2治疗恶性腹腔积液44例疗效观察

目的：观察鸦胆子油乳剂联合白细胞介素-2腹腔内注射治疗恶性腹腔积液的疗效和不良反应。方法：对44例恶性腹腔积液患者采用腹腔放置引流管放净腹腔积液或腹腔排净腹腔积液后，分为两组。每组22例，联合用药组腹腔内每次注入鸦胆子油乳剂80～100ml，白细胞介素-2300万～500万单位，单药组腹腔内注入鸦胆子油乳剂80～100ml，5～7d1次，最多4次为1个疗程，1个月后观察疗效、生活质量和不良反应。结果：联合用药组治疗恶性腹腔积液的有效率为90．90％（20／22），不良反应为发热（〈38．5℃）19例，胃肠道反应1例，腹痛3例。单药组有效率为59．09％（12／22），不良反应为发热（（38．5℃）2例，胃肠道反应2例，腹痛1例。结论：鸦胆子油乳剂联合白细胞介素-2治疗恶性腹腔积液比单用鸦胆子油乳剂近期疗效、生活质量明显提高（P〈0．05），不良反应给予对症处理后很快缓解，值得进一步推广使用。

The results of 44 patients with Malignant Peritoneal Effusion treated with Brucea Javanice Oil Emulsion Combined with Interleukine-2

Objective： To evaluate the curative effects and side effects of Brucea Javanice Oil Emulsion （B JOE） combined with Interleukine-2, in the treatment of Peritoneal Effusion by thoracic injection. Methods： 44 cases with Peritoneal Effu sion was randomly divided into two groups： combined group（22 cases）and single drug group （22 cases）.All the cases were drained pleural effusion by tube in thorax. Brucea Javanice Oil Emulsion（80-100 ml） and Interleukine-2（300-5000 000 U） in combined group, Brucea Javanice Oil Emulsion （80-100 ml） in single drug group, once every 5-7 days,4 times for a course of treatment, then the effects,quality of life and side effects were observed after one month. Results： The effective rate of combined group was 90.90%（20/22）. The side effects were fever （〈38.5℃,19 cases） and digestive sickness（1 case）, as well thoracalgia （3 cases）. The effective rate of single drug group was 59.09%（12/22）. The side effects were fever （〈38.5℃, 2 cases） and digestive sickness（2 cases）,as well thoracalgia （1 case）. Conclusion： The short-term effect and quality of life in combined group were superior to single drug group（P〈0.05）.Given the symptomatic treatment, the side effects were eased soon. It is worth of further promoting the use.
