骨原发性淋巴瘤的MR表现

目的观察骨原发性淋巴瘤的MR表现,探讨其信号特征.方法对7例骨原发性B细胞淋巴瘤行MR检查.结果所有7例均为单骨发病,累及腰椎2例,髂骨1例,髋臼1例,股骨上段1例,肱骨1例,腓骨1例.病变在T1WI呈等或稍低信号,T2WI呈等或稍高信号,注入对比剂后呈轻至中度均匀或不均匀强化.7例病灶周围均见大小不等的软组织肿块影.结论骨原发性淋巴瘤主要侵犯四肢骨,T2WI信号不高,增强扫描轻至中度强化,病灶周围均有较大软组织肿块.

MR appearance of primary bone lymphoma

Objective To study MR manifestation and signal characteristics of primary bone lymphoma. Methods Seven patients (5 males, 2 females) with primary B cell non-hodgkin's lymphoma of bone were included in this study. MR imaging were performed in all patients, nuclide bone scan were performed in 4 patients. Results All lesions affected single bone, 2 lesions involved the lumbar vertebrae, in one case each the ilium, acetabulum, femur, humerus, and fibula, respectively, were affected. Most of the lesions showed homogeneous signal intensity except 1 pathologic fracture lesion in femur and 1 center necrosis lesion in humerus. Lesions showed equal or low signal intensity on T1WI, showed equal or slightly high signal intensity on T2WI. Fat-suppression scanning showed no significant increase in signal intensity. Lesions showed slightly or moderate homogeneous or inhomogeneous enhancement after intravenous injection of contrast agent. Unequal size of soft-tissue masses were present in all cases. Conclusion Primary bone lymphoma mainly involved the extremities. Most of lesions showed homogeneous signal intensity, equal or slightly high signal intensity on T2WI, slightly or moderate enhancement after intravenous injection of Gd-DTPA and soft-tissue masses were present around all lesions.
