肩锁关节脱位手术方法探讨

目的观察运用克氏针固定、钢丝环扎、锁骨钩钢板三种方法治疗肩锁关节脱位进行切开复位内固定手术的临床效果。方法从2000年5月~2004年5月运用以上三种方法治疗了27例肩锁关节脱位的患者。平均年龄33岁,随访时间6~16个月,平均9个月。结果所有患者均获得良好复位和固定,其中再脱位1例,并发肩峰骨折1例,有5例患者针尾刺破皮肤致感染渗出。结论克氏针固定、钢丝环扎存在一定缺点,锁骨钩钢板是治疗肩锁关节脱位的一种可靠、值得推广的方法。

Study of Surgical Technique for Acromioclavicular Joint Dislocation

Objective To compare the curative effect of different operative methods for acromioclavicular joint dislocation.Methods From May 2000 to May 2004,27 cases with acromioclavicular joint dislocation treated by internal fixation separately with Kirschner pins,tension-band wire as well as clavicular hook plate were reviewed.The average age of these patients was 33 years and the average time of follow-up was 9 months(6~16 months).Results Most patients received satisfactory curative results.The postoperative complications included redislocation in 1,acromion fracture in 1 and skin infection in 5.Conclusion Some drawbacks associated with the Kirschner pins or tension-band wire used alone for treatment of acromioclavicular dislocation.While internal fixation with clavicular hook plate is a reliable method and should be applied widely.
