缺血性心肌病患者血浆心钠素分泌调控的相关因素分析

目的：探讨缺血性心肌病 (ICM)患者血浆心钠素 (ANF)分泌调控的相关因素。方法：研究组 36例 ICM患者，年龄 45～ 70(平均 55. 4± 8. 4)岁，正常对照组 40例，年龄 42～ 68(平均 53. 46± 7. 6)岁，应用超声心动图技术和放射性免疫测定技术评价 ICM患者 ANF水平变化与左房功能变化的相关性进行分析。结果： 36例 ICM患者的血浆心钠素水平与左房功能指标 LAT、 LAV、 LAP、 ANF、 LAD、 A峰及 ESS呈显著线性相关意义；与左室功能指标 LVV、 EF及 PFR呈中度相关。结论：随着心衰的加重，血浆心钠素水平有增加趋势，测定 ANF水平对于评价左房室功能异常变化有重要意义。

Analysis of causes associations of atrial natriuretic factor secretion in patients with ischemic cardiomyopathy

Objective: To analyze the atrial nutriuretic factor(ANF)secretion in patients with ischemic cardiomyopathy (ICM).Methods:The study group consisted 36 patients of ICM,the mean age was 55.4±8.4 years.Forty normal cases were in the control(NC) group, the mean age was 53.46±7.6.Plasma atrial natriuretic factor(PANF) level and left atrial function as well as the changes of hemodynamics in patients with ICM and NC group were studied by two dimensional Doppler echocardiography and radioimmunoassav.Results: The results showed that the PANF level in ICM group was higher than that in normal control(NC) group(P＜O.001).There were significant linear correlations between PANF and the indexes of left atrial function:left atrial tension(LAT),left atrial volume(LAV),left atrial pressure(LAP),left atrial ejection force(F),left atrial diameters(LAD),peak velocity A and left ventricular and systolic stress(ESS),and moderate linear correlations between PANF level and left ventricular volume(LVV),ejection fraction(EF) and peak filling rate(PFR).Conclusions: The more severe of cardiac failure, the more increase of the PANF level.To detect the level of PANF was useful in estimation of the changes of left atrial - ventricular hemodynamics in patients with ICM.
