2型糖尿病患者chemerin水平变化与下肢血管病变的相关性

目的 探讨2型糖尿病(T2DM)患者血浆chemerin水平变化及其与T2DM下肢动脉血管病变的关系。方法 选取T2DM患者80例，根据有无下肢血管病变及病变程度不同分为四组：单纯2型糖尿病无下肢动脉血管病变组16例(DM0组)；合并下肢动脉血管内膜增厚组19例(DM1组)；合并下肢动脉血管硬化组23例(DM2组)；合并下肢动脉血管狭窄组22例(DM3组)。另外选取正常健康查体者20例作为正常对照组(NC组)。用酶联免疫法检测各组受试者血浆chemerin水平,并分析血浆chemerin水平与体质量指数(BMI)、体脂成分(FTA%)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)等的关系。结果 T2DM及T2DM合并下肢动脉血管病变患者血浆chemerin水平与对照组比较显著升高，且与下肢动脉血管病变程度相关(P＜0.05)。血浆chemerin水平与BMI、FAT%、HbA1c、HOMA-IR 呈正相关(P<0．01)。结论 T2DM 患者血浆chemerin水平升高，chemerin水平升高可能与T2DM下肢动脉粥样硬化的发生发展有关。

The plasma chemerin level is related to vasculopathy of lower extremities in type 2 diabetes mellitus

Objective To investigate the levels of chemerin in the patients with type 2 diabetes mellitus(T2DM)， and evaluate the relationship between chemerin and the T2DM patients with vasculopathy of lower extremities． Methods Eighty T2DM patients and twenty healthy volunteers were selected as subjects，and the patients were further divided into four groups according to the degree of vasculopathy of lower extremities, including T2DM without vasculopathy (DM0 group), T2DM with vascular intima thickening(DM1 group), T2DM with vascular intima cirrhosis (DM2 group), and T2DM with vascular intima stenosis (DM3 group)． The plasma chemerin were measured by ELISA，and the relationship between the plasma chemerin and BMI， FTA%， HbA1c， HOMA insulin resistance index(HOMA-IR) was analyzed． Results The plasma chemerin of the patients were significantly higher than that of the control group(P＜0．05)， especially in the groups with vasculopathy of lower extremities． Correlation analysis showed that the levels of plasma chemerin were positively correlated to BMI(r=0．792)，FAT%(r=0．717)， HbA1c (r=0．418)， and HOMA-IR(r=0．684), respectively(P<0．01)． Conclusion The plasma-chemerin-in the T2DM patients are increased， and the levels may be attributed to the development of vasculopathy of lower extremities．
