E-选择素S128R基因型与糖尿病肾病的相关性

摘    要:目的探讨E-选择素基因S128R多态性各等位基因及基因型在桂北地区汉族糖尿病肾病(DN)患者中的分布频率,并分析其基因型与DN的相关性。方法采用PCR—RFLP方法检测205例DN患者(DN组)及198例健康查体者(对照组)的S128R基因型,同时检测两组相关的生化指标水平。结果DN组FPG、HbA1、TG、TC、BUN、Cr等高于对照组(P〈0.05),两组S128R基因型分布具有统计学差异(X^2=7.37,P〈0.05);RR基因型患DN的风险为ss基因型的2.34倍(OR=2.34,95%CI:1.91~2.86);两组s等位基因和R等位基因频率有统计学差异(X^2=14.73,P〈0.05,OR=2.34,95%CI:1.50~3.65),携带R等位基因的DN患者相关实验室指标水平明显高于不携带R等位基因者。结论E-选择素S128R基因多态性与DN的发病有关,其中R等位基因可能是广西桂北地区汉族人DN发病的遗传易感基因,携带R等位基因的个体可能通过影响血糖、血脂的代谢而增加DN的发病风险。

Corelation between the E-Selectin S128R gene polymorphism and type 2 diabetic nephropathy

Abstract:Objective To study the allele frequencies and genotype distribution of E-selectin gene S128R polymorphism in HaM nationality in Guangxi' s northern with type 2 diabetic nephropathy ( DN), and analyze the correlation between S128R genotype and DN. Methods PCR-RFLP way was used to detect the S128R gene polymorphism of 205 cases of DN patients and 198 cases of healthy control persons, in the same time, testing related biochemical index level. Results FPG, HbA1c, TG, TC, BUN, Cr levels in DN group were higher than those in control group(P 〈0.05). There was significantly different on the S128R genotype distribution between the two groups(x2=7.37, P 〈 0.05 ). The risk of DN patients with RR genotype was 2.34 times than that of the DN patients with SS genotype ( OR = 2.34, 95% CI: 1.91 2.86). The frequencies between the S allele and the R allele genotype were significantly different in the two groups (X2 = 14.73 ,P 〈0.05 ,OR =2.34,95% CI:1. 50 - 3.65). Clinical index levels in DN patients carried with R alleles were signif- icantly higher than those in DN patients without R alleles. Conclusions The polymorphism of E-selectin S128R genotype is associated with the onset of diabetic nehropathy. The R allele genotype may be inherited susceptibly genetic of diabetic nephropathy of HaM north of Guangxi region. The R allele carrier may add to the onset risk of DN by influencing the metabolism of blood sugar and blood lipid.
