中耳乳突结核8例分析

目的对中耳乳突结核的诊疗方案进行探讨.方法回顾性分析我科自1986～2003年共收治的中耳乳突结核8例(9耳).其中手术加抗痨治疗6例(7耳),单纯抗痨治疗2例(2耳),均有病理诊断.结果随访6个月～3年,8例(9耳)耳漏均停止,听力改善不明显,手术的6例(7耳)术腔均上皮化,耳漏停止.结论正规抗痨治疗是中耳乳突结核的基本方案,而手术可以清除中耳乳突炎性肉芽及死骨.

A clinical analysis on 8 patients with tubercular otitis media and mastoiditis

Objective To discuss the proper diagnostic program and therapy for tubercular otitis media and mastoiditis. Methods 8 patients(9 ears) with tubercular otitis media and mastoiditis, treated in our hospital from 1986 to 2003, were included in this paper for a retrospective study on their clinical data, all being diagnosed on the basis of histopathology. Among them, 6 (7 ears) were treated by a combined therapy with surgery and anti-tubercular drugs and the other 2 were treated only by anti-tubercular therapy. Result Among the 8 patients, 6 (7 ears)received operation were cured with the operating cavity epithelized, with all cases suppurative otorrhea stopped, during the following up period from 6 months to 3 years. Conclusions Although the regular anti-tubercular treatment is the main therapy for this kind of condition, surgical treatment should be helpful to clean up the inflammatory granular tissues and necrotic bones in middle ear and mastoid. Therefore,they can be used in clinic in a combined way for the treatment of tubercular lesion in middle ear and mastoid.
