Halo架固定下枕颈融合治疗陈旧性寰枢关节脱位

摘    要:目的 观察在Halo架固定下枕颈融合治疗陈旧性寰枢关节脱位的效果。方法 自 1991年~ 1998年对 19例陈旧性寰枢关节脱位患者在Halo架固定下 ,采用寰椎后弓切除、切开增厚的硬膜与切断粘连带彻底减压下 ,应用大块髂骨植于枕骨与C2或C3 棘突根部。结果 经 1~ 6年的随访 ,植骨全部获骨性愈合 ,所有患者症状明显改善。其中 12例四肢肌力、肌张力恢复正常 ;7例四肢肌力均在原基础上提高Ⅰ~Ⅱ级 ,感觉恢复正常或有明显改善 ,双下肢肌张力仍高者 5例 ,生活能自理。结论 在Halo架固定下行枕颈植骨融合术比其它术式安全效果满意

Abstract:Objective To observe the effect of occipitocervical fusion for old atlanto-axial joint dislocation under fixation of Halo frame.Methods Form 1991 to 1998,19 patients were treated by remoral of posterior arch of atlas,cutting off the thicked dura membrane of spinal cord and adhesion zone and transplantation of the ilium bulk to occipital bone and roof of spina of C 2 or C 3.Results followed up from 1 to 6 years,all bone grafts have bony fusion.the symptoms of patients were improved obviously.Muscular strength and muscular ton in 12 cases were restored to normal.Muscular strength in 7 cases was increased from one to two degrees.The sensory function was restored to normal or improved obviously.The muscular ton of double lower limbs in 5 case in still high,but the patients can live by himself.Conclusions Occipitocervical fusion for atlanto-axial joint dislocation by Halo frame is safer and more effective than other surgical methods.
