食管基底细胞样鳞状细胞癌临床病理分析

目的探讨食管基底细胞样鳞癌(BSC)的临床病理特点.方法回顾分析10年间食管癌手术病理档案,对符合基底细胞样鳞癌的病例行免疫组化标记.结果2*!960例食管癌中10例为食管基底细胞样鳞癌,占0.3%.其中8例曾误诊为低分化鳞癌、腺鳞癌、腺样囊性癌和小细胞癌.肿瘤主要由核大浆少的基底细胞样癌组织与多少不等的鳞癌或(和)原位鳞癌构成,两者间无移行.1例部分细胞免疫组化癌胚抗原阳性,2例局部癌组织突出素、嗜铬素阳性,1例灶性角蛋白17、角蛋白8阳性;无一例表达S100和肌动蛋白.结论食管基底细胞样鳞癌主要由基底样细胞组成,并具有鳞癌的变异型,具有高侵袭性又易在病理上误诊,因而需要对其临床和病理特征进行透彻研究和进一步提高认识.

Clinicopathologic analysis on basaloid squamous carcinoma of the esophagus

Objective To investigate clinicopathologic features of basaloid squamous carcinoma of esophagus.Methods The slides of all esophageal carcinomas from 1990 to 2000 were reviewed through which the cases of basaloid squamous carcinoma were marked immunohistochemically.Results There were ten cases(0.3%) of basaloid squamouse carcinoma in 2?960 cases of esophageal carcinoma,Among which 8 had been misdiagnosed as poorly differentiated squamous cell carcinoma,adenosquamous carcinoma, adenoid cystic carcinoma and small cell carcinoma.The tumors were mainly composed of basaloid cells with big nuclei,scanty cytoplasm and a higher nucleus/cytoplasm ratio and no transition between the two elements. Immunohistochemical studies showed CEA positive in one case,SYN and CGA positive in two cases,CK17 and CK8 positive in one case.None of the cases expressed S100 and Actin.Conclusions Basaloid squamous carcinoma is a variant of squamouse carcinoma with highly aggressive,which is mainly composed of basaloid cells and easy to be misdiagnosed histologically.Its clinical and pathological features should be studied thoroughly.
