摘    要:中性粒细胞弹性蛋白酶(NE)是中性粒细胞原代颗粒中的一种主要蛋白酶,参与支气管扩张复杂病理的发生、发展,具有调节炎症反应、控制感染、增加黏液分泌、降解细胞外基质和细胞因子及损害免疫系统等多种生物学功能,与疾病预后有关,可作为临床治疗结果的预测因子和可能的治疗靶点。尚未有大量高级别临床研究证明内源性及外源性NE抑制剂对支气管扩张的疗效。中医药发挥其多靶点、多途径、精准施治等作用干预NE介导的支气管扩张,在改善临床症状、减少急性加重次数、减轻气道炎症反应等方面具有优势。该文综述NE在支气管扩张中的主要生物学功能,以及中医药干预NE介导的气道炎症的进展,期望为临床和科研提供有价值的参考依据。

Abstract:Neutrophil elastase (NE) is a major protease in neutrophil primary granules, involved in the occurrence and development of complex pathology of bronchiectasis, with a variety of biological functions such as regulating inflammatory response, controlling infection, increasing mucus secretion, degrading extracellular matrix and cytokines, and damaging the immune system, and has important correlations with disease prognosis, NE can be used as a predictor of clinical treatment outcome and a possible therapeutic target. There are not a large number of high-level clinical studies demonstrating the efficacy of endogenous and exogenous NE inhibitors in bronchiectasis. Traditional Chinese medicine plays its role in multi-target, multi-pathway and precise treatment to intervene in NE-mediated bronchiectasis, which has advantages in improving clinical symptoms, reducing the number of acute exacerbations, and reducing airway inflammatory response. This article analyzes the main biological functions of NE in bronchiectasis and the progression of traditional Chinese medicine intervention in NE-mediated airway inflammation, hoping to provide valuable reference for clinical and scientific researchers.
