谷胱甘肽-S-转移酶基因多态与原发型肝癌的Meta分析

摘    要:目的:探索谷胱甘肽-S-转移酶(GSTM1、GSTT1、GSTP1)基因多态与原发型肝癌遗传易患性的关系。方法:采用Meta分析方法对国内外1994--2004年关于谷胱甘肽-S-转移酶基因多态与原发型肝癌易患性的研究文献进行综合定量分析。结果:共收集相关文献18篇,累计病例1407例,对照2044例。GSTM1空白基因型可能与原发性肝癌有关,OR值为1.40(95%CI:1.22-1.62);GSTM1和GSTT1中至少有一个空白基因型的OR值为2.05(95CI:%1.14-3.67),亦提示可能与原发型肝癌的遗传易患性有关;而GSTT1空白基因型、GSTP1突变基因型未显示与原发型肝癌相关,合并的OR值分别为1.13(95%CI:0.77-1.65)和0.68(95%CI:0.47~0.99)。结论:谷胱甘肽-S-转移酶基因多态与原发性肝癌遗传易患性的关系各有不同,GSTM1单项空白基因和GSTM1、GSTT1混合空白基因与原发性肝癌有统计学联系,可能是原发性肝癌的易患因素;GSTT1空白基因型与原发性肝癌未显示有统计学联系,而GSTP1的突变基因型可能是原发性肝癌的保护因素。

Abstract:Objective:To study the association of genetic polymorphism of glutathione S-transferase(GSTM1,GSTT1,GSTP1) and their susceptibility to primary liver cancer.Methods: The results from 18 epidemiological studies on the relationship between glutathione S-transferase genetic polymorphism and susceptibility to primary liver cancer from 1994 to 2004 were analyzed synthetically by Meta-analysis.Results:A total of 1 407 cases and 2 044 controls from 18 studies was included.The results showed a relationship between GSTM1 null genotype and susceptibility to primary liver cancer,the pooled odds ratio(OR) values was 1.40(95%CI:1.22-1.62);the relationship also was found in the study on at least one null genotype in GSTM1 and GSTT1 OR=2.05(95%CI: 1.14-3.67).The null genotype of GSTT1 OR= 1.13(95%CI:0.77-1.65) and the mutated genotype of GSTP1 OR=0.68(95%CI:0.47-0.99) did not show a statistic relationship with susceptibility to primary liver cancer.Conclusion:Our results by Meta-analysis may support the positive associations of genetic polymorphism of glutathione S-transferase M1,M1 or T1 and susceptibility to primary liver cancer.None of the null genotype of GSTT1 and the mutated genotype of GSTP1 may be a risk factor of primary liver cancer.
