脑海绵状血管瘤临床和影像学表现

目的 分析脑海绵状血管瘤(CA)的临床和影像学特点.方法 对57例经手术病理证实的脑CA的临床和CT、MRI及DSA影像学资料进行分析,其中脑内型CA 48例,脑外型CA 9例.结果 脑内型CA的CT平扫表现为混杂颗粒状较高密度,MRI多呈短或等T1、稍长T2混杂信号且周边常伴明显短T2信号环,增强扫描病灶轻、中度强化或无强化,脑内型CA DSA检查未见异常染色.脑外型CA位于鞍旁区,CT平扫为等密度或稍高密度;MRI平扫为等T1或稍长T1,明显长T2信号,增强后均明显强化,呈“揉面团“样;DSA检查可见肿瘤染色.结论 脑CA的CT和MRI影像表现各有特征性,综合CT、MRI以及DSA所见有助于正确的临床诊断.

Clinical and imaging features of cerebral cavernous angioma

Objective] To analyze the clinical and imaging features of cerebral cavernous angioma(CA).Methods] The clinical and CT,MRI,DSA imaging data of 57 cases of cerebral CA were collected and studied.All cases were proved by surgical and pathologic findings.Among the cases,48 were intracerebral CA and 9 were extracerebral CA.Results] Intracranial CA presented as slightly-high density with a granulo mixed center on plain CT,heterogeneous intense on MRI.Most of the intracerebral Cas appeared as hyperintense or isointense signal on T1WI and a mixed-hyperintense core surrounded by a hypointense halo on T2WI.and negative on DSA.Both on contrast-enhanced CT and Gd-DTPA enhanced MRI,the intracerebral CA demonstrated mild,slight enhancement or no enhancement.9 cases extracerebral CAs located in parasellar region,which presented as isointensity or slightly-hyperintensity on plain CT and isointense signal or slightly hypointense signal on T1WI,but markedly hyperintense on T2WI.All extracerebral CA lesions demonstrated intensive enhancement on enhanced CT or Gd-DTPA enhanced MRI,which appeared as a "knead dough".Conclusion] Intracranial and extracerebral CA is of specific feature on CT and MRI.The combination of CT,MRI and DSA is helpful for the diagnosis of the disease.
