生长抑素对重症急性胰腺炎患者血小板参数影响的探讨

目的　探讨重症急性胰腺炎患者血小板参数的变化特点及生长抑素治疗的影响。方法　应用血细胞自动分析仪检测轻症急性胰腺炎 (MAP)和重症急性胰腺炎 (SAP)患者不同时期血小板计数 (PL T)、血小板容积 (PCT)、平均血小板体积(MPV )和血小板分布宽度 (PDW)。结果　与 MAP组患者相比 ,SAP患者入院时外周血 PL T、PCT明显下降 (P<0 .0 0 1) ,MPV和 PDW有显著升高 (P<0 .0 5 ) ;SAP患者死亡组与存活组相比 ,PL T、PCT明显下降 (P<0 .0 1) ,MPV和 PDW有明显升高 (P<0 .0 1) ;与常规治疗组相比 ,生长抑素治疗组 SAP患者 PL T、PCT明显上升 (P<0 .0 1) ,MPV和 PDW有明显下降 (P<0 .0 5 )。结论　 SAP与 MAP患者血小板参数的变化程度明显不同 ,可反映病情变化程度。生长抑素治疗后 PL T、PCT有较快的提升 ,血小板的活性降低 ,对 SAP患者具有一定的治疗和预防作用。

Effects of Somatostatin on the Platelet Parameter of Patients with Severe Acute Pancreatitis

Objective To investigate the change characteristics of the platelet parameter in the patients with MAP and the patients with SAP and the therapeutic effects of somatostatin on platelet parameter.Methods The autocytometer was used to check PLT,PCT,MPV and PDW in different periods in the patient with MAP and the patients with SAP.Results There were obviously decrease (P<0.001)in the PLT and PCT in the patients with SAP to the patients with MAP,when they were admitted to hospital,meanwhile MPV and PDW evidently increase(P<0.05).The death patients with SAP had lower PLT and PCT to te cured patients (P<0.01),but the MPV and PDW were higher(P<0.01).Compare with routine treatment ,after a week,the treatment of Somatostatin made PLT and PCT rise sharply(P<0.01).moreover MPV and PDW markedlyu decrease (P<0.05).Conclusions There was clear difference between patients with MAP and patients with SAP in the change characteristics of the platelet parameter which can ,reflect the change of patient's severe condition.The treatment of somatostatin has obvious influence on PLT and PCT,but it can tangibly decrease the activity of the platelet and has certainly therapeutic and preventive effects.
