右美托咪定在困难气道患者纤维支气管镜插管中的麻醉效果

目的:探讨困难气道患者纤维支气管插管中应用右美托咪定麻醉的效果。方法:64例择期行纤支镜插管的困难气道患者随机分为观察组与对照组,各32例。对照组应用表面麻醉联合瑞芬太尼麻醉,观察组在此基础上加用右美托咪定麻醉,比较两组的麻醉效果。结果:两组给药后(T_1)、插管时(T_2)及插管后(T_3)时MAP均较入室时(T_0)显著提高(P0.05),观察组显著低于对照组(P0.05):T_1、T_2、T_3时观察组HR显著低于对照组(P0.05):T_1~T_5时观察组的Ramsay评分显著高于对照组(P0.05)。结论:右美托咪定应用于困难气道患者纤支镜插管能够提高麻醉镇静效果,减少应激反应,提高插管安全性与成功率,值得推广应用。

Dexmedetomidine given anesthesia in patients with difficult airways fiber bronchoscopy Intubation

Objective:To investigate the difficulties fiber bronchial airway intubation application dextromethorphan care given anesthetic effect microphone. Methods:64 cases of difficult airways in patients undergoing elective fiberoptic intubation were randomly divided into observation group and control group,32 ca-ses. Application of surface anesthesia combined group remifentanil anesthesia,observed in this group based on the use of dexmedetomidine given anesthesia,the anesthetic effect between the two groups. Results:The two groups after dosing(T1 ),intubation(T2 )and after intubation(T3 )when compared MAP burglary (T0 )significantly increased(P ﹤ 0. 05),the observation group was significantly lower than the control group(P ﹤ 0. 05);T1 ,T2 ,T3 HR during the observation group was significantly lower than the control group(P ﹤ 0. 05);T1 - T5 Ramsay score when the observation group was significantly higher(P ﹤ 0. 05). Conclu-sions:dexmedetomidine given applied airway fiberoptic intubation difficulty can be increased sedative effect of anesthesia,reduce stress,improve safety and suc-cess rate of intubation,should be widely applied.
