摘    要:目的利用双能量CT(dual-energYcomputedtomograplay,DECT)检测关节痛患者尿酸盐结晶沉积情况,探讨DECT对单独存在或合并痛风性关节炎患者的检出能力。方法对34例关节痛患者进行DECT扫描,根据血尿酸浓度是否超出正常范围将其分为病例组(n=16)及对照组(11=18),图像由DECTGOUT软件处理后,由两名医师独立进行评价,比较两纽尿酸盐沉积的差异,进一步评价患者血尿酸浓度、病程长短与病灶数目的相关性。结染病例组16例患者中均发现有绿色颗粒状尿酸盐沉积,对照组均未见尿酸盐沉积,两组差异具有统计学意义(P=0.000):患者血尿酸浓度、病程长短与病灶数量无相关性(P〉0.05)。结论DECT尿酸盐结晶成像技术能够是敏的检出单独存在或合并痛风性关节炎的患者,对早期、无创性诊断痛风性关节炎具有重要应用价值。

Application of Dual Energy Computed Tomog- raphy for Detecting Uric Acid Deposition

Abstract:Objective To explore the ability of dual energy computed tomography( DECT )in detecting the arthralgic patients with gout alone or with other diseases combind by using DECT to detect the uric acid (UA) deposition. Methods DECT was performed for the total of 34 patients with arthralgia who were divided into two grous according to UA level in the blood. Color coding was used to display the localization of UA deposition. Images were reviewed independently by two trained radiologists. The differences of UA deposi- tion between the two groups were compared. Results With DECT, UA deposits were observed in the case group, whereas none in the control group, which were significantly different between them( P = 0.000);There were no correlation between UA level in the bloodi¢the course of althralgia and the number of lesions. Conchtsions DECT allows the visualization of UA deposition in arthralgic patients with gout alone or with other diseases combined, which provides noninwlsive theoretical evidence for their early rehabilitation.
