内科非手术患者诊断相关组分组研究

目的:为了解决我国当前医疗支付制度不完善所造成就医难问题,突破单病种支付的局限,利用诊断相关组方法对内科非手术病例进行分组研究,为未来的支付制度设计提供参考。方法:通过与MS-DRGs(2010)比对建立基本组,住院费用为目标变量,其他影响费用因素为分类变量,SPSS 17.0建立决策树模型进行细分组,SAS 9.1进行数据处理及细分组效果指标的计算。结果:建立14个病种共40个细分组,各基本组决定系数(R2)从0.027到0.269。结论:分组组间异质性增加,符合DRGs分组要求,分组规则简便且考虑了临床适用性。
