甲状腺乳头状癌颈淋巴结转移规律的研究

本文报告甲状腺乳头状癌颈淋巴结清除术标本104例系统性病理检查结果。对颈部淋巴结转移的总阳性率、转移部位及各区淋巴结受累程度等,作了详细地记录。结果表明,颈部各区淋巴结中,除颏下区外,其余均可见癌转移,但转移的机率及受累程度不一。气管区及颈内静脉区癌转移机率最大,分别为58.7％及55.8％;受累程度,气管区淋巴结中癌性淋巴结占47.3％,颈内静脉区为32.9％,颌下区最低,仅为0.9％,颏下区无一淋巴结转移。提示气管区及颈内静脉区是颈淋巴清除术的重点部位。

Study on the Laws Governing Metastasis to the Neck Lymph Nodes Seen in Papillocarcinoma of the Thyroid Gland

Systematic pathologic examination results of the specimens of 104 cases of dissection of the neck lymph nodes (NLD) performed for papillocarinoma of the thyroid gland is reported in this paper. The total positive rate of metastasis to the cervical lymph nodes, the sites of metastasis as well as the extent in which the lymph nodes of various regions were involved were recorded in detail. Results showed that of the lymph nodes of the various regions of the neck, except in the submental region, carcinometastasis could be seen in all the other areas. However, the metastatic probability and degree of involvement varied. Specifically, the largest cancerous metastatic probability was seen in the region of the trachea (58.7%)and in that of the vena jugularis interna (55.8%). As to the extent of involvement,the cancerous lymph nodes seen in the regions of the trachea, the vena jugularis interna and the submaxilla accounted for 47.3%, 32.90% and 0.9% respectively. Not a single metastasis was detected in the area beneath the chin. These findings suggest that the regions of the trachea and the vena jugularis interna constitute the key sites of operation for NLD.
