剪切波弹性成像鉴别乳腺良恶性肿瘤的价值

摘    要:目的应用剪切波弹性成像(SWE)分析乳腺肿瘤的成像特点,定量及定性评价SWE对乳腺良、恶性肿瘤的鉴别诊断价值。方法收集我院病理证实的乳腺肿瘤女性患者52例,共69个病灶,行剪切波弹性成像检查,采集肿瘤二维及SWE图像,获得乳腺肿瘤的弹性模量值E(kPa):平均值(Emean)、最小值(Emin)、最大值(Emax)和标准差(SD),对照病理结果,分析良、恶性肿瘤SWE成像特点,比较良、恶性肿瘤的弹性模量值。比较二维与弹性图像测量良、恶性肿瘤最大径。结果 (1)乳腺良性肿瘤SWE典型表现:蓝色为主,颜色较均一;恶性肿瘤SWE典型表现:红色为主,颜色较杂乱。(2)乳腺良性肿瘤的Emean、Emax及SD均低于恶性肿瘤,Emin高于恶性肿瘤,P<0.05。(3)弹性图像测量良性肿瘤的最大径与二维图像测量比较,差异无统计学意义,P>0.05;弹性图像测量恶性肿瘤的最大径大于二维测量,P<0.05。结论乳腺良、恶性肿瘤有各自典型的SWE成像特点,鉴别良、恶性有较好的价值。

Abstract:Objective Analysis of the features of the shear wave elastography (SWE) imaging about the breast tumors, to application of quantitative and qualitative evaluation of SWE in the diagnosis of breast tumors. Methods 52 patients with breast tumor pathology confirmed were collected, a total of 69 lesions. The E-mean,E-min,E-max and SD of the breast tumors were obtained using SWE. Analyze the SWE imaging characteristics of the breast tumors, contrasting with the pathological results. Results (1) The typical image of the benign tumors was mainly blue and the color uniform. The typical image of the malignant tumors was mainly red and the color clutter. (2) The E-mean, E-max and SD of the benign tumors were lower than those of the malignant tumors, the E-min of the benign tumors were higher than that of malignant tumors, P&lt;0.05. (3)The largest diameter of using elastic image measurement of the malignant tumor was greater than the application of 2D measurement, P&lt;0.05. Conclusion Benign and malignant breast tumors have their typical SWE imaging features. SWE is helpful to the diagnosis of breast cancer.
