氧化型高密度脂蛋白与冠状动脉粥样硬化性心脏病的 相关性分析

摘    要:目的 研究冠状动脉粥样硬化性心脏病(CHD)患者血浆中氧化型高密度脂蛋白(ox-HDL)水平与冠状动脉病变程度(Gensini评分)之间的相关性,ox-HDL与高密度脂蛋白胆固醇(HDL-C)的比值(ox-HDL/HDL-C)相较HDL-C对血浆HDL真实水平的代表性。方法 选取2018年7月~2019年2月于新乡医学院第一附属医院心内科二病区住院行冠状动脉造影被确诊为冠心病的患者84例为实验组,另选取冠状动脉造影正常者44例为对照组,通过酶联免疫吸附剂(ELISA)检测所有研究对象血浆中ox-HDL水平,应用Pearson相关性分析实验组内ox-HDL、ox-HDL/HDL-C分别与Hcy、冠状动脉粥样硬化病变程度(采用Gensini评分评价)的相关性。结果 实验组ox-HDL水平高于对照组,差异具有统计学意义(P<0.05);ox-HDL与Gensini评分呈现明显的正相关(r=0.335,P<0.01);oxHDL/HDL-C与Gensini评分之间呈显著正相关(r=0.819,P<0.01);多因素Logistic回归分析结果表明ox-HDL水平与冠心病的病变程度相关。结论 ox-HDL是冠心病的危险因素,应用ox-HDL及ox-HDL/HDL-C可以更好的评价冠状动脉病变程度以及冠心病患者血浆HDL-C的真实水平,ox-HDL可能会成为动脉粥样硬化的新的危险因素和治疗的新靶点。

Abstract:Abstract:Objective To investigate the correlation between plasma oxidized high-density lipoprotein (ox-HDL) levels and coronary artery lesions (Gensini score) in patients with coronary atherosclerotic heart disease (CHD), ox-HDL and high. The ratio of density lipoprotein cholesterol (HDL-C) (ox-HDL/HDL-C) is representative of the true level of plasma HDL compared to HDL-C.Methods From July 2018 to February 2019, 84 patients with coronary heart disease who underwent coronary angiography in the Department of Cardiology of the First Affiliated Hospital of Xinxiang Medical College were enrolled in the study group. The patients with normal coronary angiography were selected. For example, the ox-HDL levels in plasma of all subjects were detected by enzyme-linked immunosorbent assay (ELISA). Pearson correlation analysis was used to analyze ox-HDL, ox-HDL/HDL-C and Hcy, coronary artery respectively. Correlation of the extent of atherosclerotic lesions (evaluated using the Gensini score).Results The ox-HDL level in the experimental group was higher than that in the control group,the difference was statistically significant (P<0.05). There was a significant positive correlation between ox-HDL and Gensini score (r=0.335,P<0.01); oxHDL/HDL-C and Gensini there was a significant positive correlation between the scores (r=0.819,P<0.01). Multivariate logistic regression analysis showed that the level of ox-HDL was correlated with the degree of coronary heart disease.Conclusion ox-HDL is a risk factor for coronary heart disease. The use of ox-HDL and ox-HDL/HDL-C can better evaluate the extent of coronary artery disease and the true level of plasma HDL-C in patients with coronary heart disease. ox-HDL may become an artery. New risk factors for atherosclerosis and new targets for treatment.
