摘    要:目的 通过三维CT对发育性髋关节发育不良(DDH)儿童髋关节进行三维重建,探讨三维CT对DDH患儿的临床价值。方法 收集2016年1月~2018年12月重庆医科大学附属儿童医院进行术前三维CT重建的DDH患儿12例,在三维重建模型上对患儿髋关节的相关结构进行测量。结果 通过对12例患儿术前髋关节三维重建模型进行观察与测量,患儿平均髋臼缺损宽度为(39.06±3.11)mm,平均髋臼缺损深度为(8.69±0.57)mm。选择缝匠肌骨瓣修补患儿髋臼缺损,术后髋臼缺损皆获得明显改善。结论 三维CT能够从多个方位观察髋关节结构,更直观、更全面的观察到髋关节形态学的变化,在三维CT模型上可直接测量髋臼缺损的大小,更有效的指导手术选择及临床工作。

Abstract:Objective To investigate the clinical value of three-dimensional CT in children with DDH by three-dimensional reconstruction of the hip joint in children with developmental hip dysplasia (DDH). Methods 12 children with DDH who underwent preoperative three-dimensional CT reconstruction from January to December 2018 in Chongqing Children's Hospital of Chongqing Medical University were enrolled. The relevant structures of hip joints were measured in a three-dimensional reconstruction model. Results The average acetabular defect width (39.06±3.11) mm and the average acetabular defect depth (8.69±0.57) mm were observed and measured in 12 children with preoperative hip three-dimensional reconstruction model. Choosing sartorius musculoskeletal flaps was used to repair acetabular defects in children,and postoperative acetabular defects were significantly improved. Conclusion Three-dimensional CT can observe the hip joint structure from multiple orientations, and the morphological changes of the hip joint can be observed more intuitively and comprehensively. The size of the acetabular defect can be directly measured on the three-dimensional CT model, which can guide the surgical selection and clinical practice more effectively jobs.
