心肌酶谱、BNP、MYO及cTnⅠ联合检测诊断急性心梗的价值

摘    要:目的 分析心肌酶谱肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、谷草转氨酶(AST)、α-羟丁酸脱氢酶(α-HBD)、肌酸激酶(CK)]、血浆脑钠肽(BNP)、肌红蛋白(MYO)及肌钙蛋白(cTnⅠ)联合检测诊断急性心梗(AMI)的价值。方法 选取2017年6月~2019年5月于我院治疗的急性胸痛患者82例,将其分为非AMI组(42例)和AMI组(40例)。检测并观察两组心肌酶谱、BNP、MYO及cTnⅠ水平,计算各指标单项及联合诊断AMI的敏感度、特异度。结果 AMI组心肌酶谱、MYO、BNP、cTnⅠ水平均高于非AMI组,差异有统计学意义(P<0.05);心肌酶谱+MYO+BNP+cTnⅠ诊断AMI的敏感度(97.50%)高于各单项指标(72.50%、70.00%、80.00%、77.50%),诊断特异度(92.86%)高于MYO(66.67%),差异有统计学意义(P<0.05)。结论 心肌酶谱、cTnⅠ、MYO、BNP联合检测有助于提升AMI诊断准确率,可为临床诊断及尽早开展治疗提供参考依据。

Abstract:Objective To analyze myocardial enzymes Creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), aspartate aminotransferase (AST), α-hydroxybutyrate dehydrogenase (α-HBD), creatine The value of combined detection of kinase (CK)], plasma brain natriuretic peptide (BNP), myoglobin (MYO) and troponin (cTnI) in the diagnosis of acute myocardial infarction (AMI). Methods 82 patients with acute chest pain who were treated in our hospital from June 2017 to May 2019 were enrolled and divided into non-AMI group (42 cases) and AMI group (40 cases). The levels of myocardial enzymes, BNP, MYO and cTnI were measured and observed, and the sensitivity and specificity of each index and combined diagnosis of AMI were calculated.Results The levels of myocardial enzymes, MYO, BNP and cTnI in the AMI group were higher than those in the non-AMI group,the difference was statistically significant (P<0.05). The sensitivity of myocardial enzymes+MYO+BNP+cTnI in diagnosing AMI (97.50%) was higher than that of individual indicators (72.50%, 70.00%, 80.00%, 77.50%).The diagnostic specificity (92.86%) was higher than MYO (66.67%),the difference was statistically significant (P<0.05).Conclusion The combined detection of myocardial enzymes, cTnI, MYO and BNP can improve the diagnostic accuracy of AMI, and provide a reference for clinical diagnosis and early treatment.
