局部阻滞加弯形针刀对屈指肌腱狭窄性腱鞘炎的疗效

目的 分析在屈指肌腱狭窄性腱鞘炎的治疗中应用局部阻滞加弯形针刀的疗效。方法 选取我院2017年10月~2018年10月收治的84例（126指）屈指肌腱狭窄性腱鞘炎患者，随机分为对照组和观察组，每组42例（62指）。对照组予以局部阻滞治疗，观察组在对照组基础上添加弯形针刀疗法。对比两组治疗前后的疼痛变化，治疗后的手指活动度及治疗效果。结果 治疗后两组的视觉模拟疼痛评分（VAS）均较治疗前降低，观察组治疗后VAS评分为（1.12±0.30）分低于对照组的（2.01±0.50）分，差异有统计学意义（P＜0.05）；观察组的手指总主动活动指数（TAM）优良率（89.06）和治疗有效率（95.31）分别高于对照组的74.19%、82.26%，差异均有统计学意义（P＜0.05）。结论 在屈指肌腱狭窄性腱鞘炎患者中应用局部阻滞加弯形针刀更利于减轻疼痛，改善手指活动度，提高治疗效果。

Therapeutic Effect of Local Block and Curved Needle Knife on Tenosynovitis of Flexor Tendon

Objective To analyze the efficacy of local block and curved needle knife in the treatment of flexor tendon stenosis tenosynovitis.Methods 84 patients (126 fingers) with flexor tendon stenosis tenosynovitis admitted to our hospital from October 2017 to October 2018 were randomly divided into control group and observation group, with 42 cases (62 fingers) in each group.The control group was treated with local block, and the observation group was given a curved needle knife therapy on the basis of the control group. The pain changes before and after treatment in the two groups, the degree of finger movement after treatment and the therapeutic effect were compared.Results The visual analogue pain score(VAS) of the two groups was lower than that before treatment.The VAS score of the observation group was (1.12±0.30) and the score of the control group was lower than that of the control group (2.01±0.50),the difference was statistically significant (P<0.05); the total active activity index (TAM) of the observation group (89.06) and the effective rate of treatment (95.31) were higher than that of the control group, 74.19% and 82.26%, respectively, the difference was statistically significant(P<0.05).Conclusion The application of local block and curved needle knife in patients with flexor tendon stenosis tenosynovitis is more beneficial to reduce pain, improve finger mobility and improve treatment effect.
