颈髓室管膜瘤的诊断与治疗

目的探讨颈髓室管膜瘤的诊断及手术治疗特点。方法分析32例颈髓室管膜瘤的临床表现、影像学特征和手术治疗结果。结果依据颈髓管膜瘤的MRI表现能作出定性诊断者占81.2%。32例均行手术治疗,获全肿瘤切除30例,次全肿瘤切除2例。所有患者症状、体征均明显改善。结论依据MRI表现颈髓管膜瘤的术前定性率高,但仍需与胶质细胞瘤、海绵状血管瘤、脊髓炎等鉴别。显微手术和肿瘤全切除是获得满意疗效的关键。

Diagnosis and treatment of cervical spinal cord ependymoma

Objective To explore the diagnosis and treatment of cervical spinal cord ependymoma.Methods Thirty-two patients with cervical spinal cord ependymoma were analyzed in terms of clinical manifestations,imageological and operational outcomes.Results 81.2% of pre-operational diagnosis was right in MRI.All the 32 patients received operations.Among them,30 patients received total resection with others receiving subtotal resection.All the patients were alleviated significantly after operation.Conclusion Atypical cervical spinal cord ependymomas should be differentiated from astrocytoma,hemangioblastoma and myslitis.MRI is helpful to achieve accurate diagnosis.The key points for acquiring satisfactory therapeutic outcome are the application of microsurgical techniques and total resection of the tumor.
